Does your child need a referral for therapy or to see a specialist?
After you confirm with your own insurance company that you need a referral, confirm that they are on your insurance.
To help us get the referral done as quickly as possible, it can help if as soon as you know that you need a referral, you call (leave a message on the nurse refill line) and let us know:
- your child’s name and date of birth
- the reason for the referral
- the date of your child’s appointment
- your insurance information
- the specialist’s name, provider ID number, and NPI number
- the specialist’s address and phone number
- your call back information
And please provide at least 72 hours (3 business days) for the referral to be done.
